3. 50-year-old woman with hand tingling (Case 26, pg. 224)
4. 33 -year-old man with back pain (Case 27, pg. 230)
5. 19-year-old with urethral discharge (Case 31, pg. 260)
Attached please find: – A sample which is exactly how it needs to be done- Checklist which should be applied to each case study – Documentation/ Patient Encounter which is the publish form to use for each case study.
